The
Home Delivered Meals Program provides nutritionally balanced
meals to individuals who are 60 years of age or older, the spouse
of someone over the age of 60 or an individual with disabilities
who resides in a non-institutional household with a person who
is eligible for and receiving Home Delivered Meals.
There is no
income eligibility requirement for this program. However, the
individual who receives Home Delivered Meals must be homebound
( does not leave their home under normal circumstances), physically
or mentally unable to attend a Congregate Meal Site or unable
to adequately prepare their own meals.
Each recipient
of the program will receive a comprehensive assessment, which
will be conducted by a Case Manager from the Commission on Aging.
The assessment will include basic personal information on the
recipient, as well as their functional status, a history of their
supporting resources and finally a recording of their identified
needs.
The Home Delivered
Meals Program is provided to the recipient on a donation basis.
At the current time the requested donation is $2.25 per meal.
If the recipient is unable to pay the donation the Home Delivered
Meal Program will not be withdrawn. The program is based on need,
not the client's ability to pay.

The Isabella
County Commission on Aging manages the Congregate Meals Program
in both Isabella and Gratiot Counties.
The Congregate
Meals Program provides nutritionally balanced meals to individuals
who are 60 years of age and older, or the spouse of someone over
the age of 60.
The meals are
provided at twelve Congregate Meal Sites throughout our two county
service area. Our meal sites promote better health through improved
nutrition, social activities and educational opportunities.
The Congregate
Meals Program is available to Older Adults on a donation basis.
At the current time the requested donation is $2.25 per meal.
If the Congregate Meal participant is unable to pay the
requested donation rate, they can still take part in the program
and receive the meals. The program is based on need, not
the participant's ability to donate.
