Case Management
The Case Management Program is the central entry point for the
provision of services to the majority of programs offered through the
Isabella County Commission on Aging. The Case Managers work with Older
Adults over the age of 60 and their family members to maintain the
independence of the Older Adult. The Case Managers with the client
and/or family will determine the needs of the individual through a
comprehensive assessment and then develop a service plan. The service
plan will identify programs and services that will assist the Older
Adult. The Case Manager will then communicate with a variety of
community agencies and organizations to arrange for those services. The
Case Managers will assist the clients and family to assure client's
health and safety, the quality of care and satisfaction with services.
The Case Managers will advocate for the Older Adult and their family
members on a variety issues. On a daily basis they deal with such issues
as Medicare, Medicaid, supplemental insurance and nursing home
placement.
The Case Management Program is provided to the Older Adult on a
donation basis. If the client is unable to donate, services will not be
denied. The program is based on need, not on the client's ability to
donate.

Homemaking Program
The Homemaking Program is the provision of light household tasks to
maintain an adequate living environment for Older Adults with functional
limitations. Homemaking tasks include: laundry, ironing, and light
housekeeping.
The Homemaking Program is provided to the Older Adult on a donation
basis. The requested donation is based on a sliding scale according the
clients level of income. If the client is unable to donate, services
will not be denied. The program is based on need, not on the client's
ability to donate.

Personal Care Program
The Personal Care Program provides a Certified Nurse Aide or trained
direct care worker to assist Older Adults with activities of daily
living. These activities include assistance with bathing, dressing,
grooming, toileting, transferring, eating and ambulation.
The Personal Care Program is provided to the Older Adult on a
donation basis. The requested donation is based on a sliding scale
according the clients level of income. If the client is unable to
donate, services will not be denied. The program is based on need, not
on the client's ability to donate.

Respite Care Program
The Respite Care Program utilizes a Certified Nurse Aide or trained
direct care worker for the provision of companionship, supervision
and/or assistance with activities of daily living for Older Adults with
mental or physical disabilities in the absence of the primary
caregiver(s). This program allows "time away" for the caregiver and
special companionship for the care recipient.
The Respite Care Program is provided to the Older Adult on a donation
basis. The requested donation is based on a sliding scale according the
clients level of income. If the client is unable to donate, services
will not be denied. The program is based on need, not on the client's
ability to donate.

Caregiver Training Program
The Caregiver Training Program is a service that is designed to
provide caregivers support with the care of their disabled family
members. This training and educational program covers caregivers under
the following categories:
1. Caregivers 60 or older who care for a family member 60 or older
2. Caregivers 60 or older who care for a family member under 60
3. Caregivers under 60 who care for a family member 60 or older.
The training and educational sessions may take place in a variety of
settings and could include topics such as medication management, how to
care for someone who is on bed rest, fall prevention, locating
resources, managing stressful situations, or symptoms of depression and
dementia. The caregivers will be able to select from these and other
topics that may be of interest to them. The caregiver trainer will then
meet with the caregiver(s) to provide information on the topic of
choice. Also available to caregivers is a monthly caregiver support
group.
The Caregiver Training Program is provided on a donation basis. If

Other Available Programs
Unmet Needs - This program is totally funded by the Saginaw / Chippewa Tribal Unit. With it the Commission on Aging is able to assist Older Adults with expenses such as: prescription medications, utility bills, minor home repairs or medical expenses. The Unmet Needs Program is limited to individuals whose income falls below 150% of the Federal poverty guidelines. Consideration is also given to individuals assets and other living expenses.
Medicare-Medicaid & other insurance
– The agency social workers are available to assist with questions
concerning Medicare, Medicaid and other insurances. Information is
available for eligibility for Medicare D, supplemental insurance,
Medicaid and other insurance questions.
Long Distance Care Giving – Many caregivers find themselves living away from their loved ones. There are times when caregivers must locate resources in a different county or state. The social workers are able to assist caregivers in finding resources that will make their loved ones safe and help with the long distance care giving.
