The
Case Management Program is the central entry point for the provision
of services to the majority of programs offered through the Isabella
County Commission on Aging. The Case Managers work with Older
Adults over the age of 60 and their family members to maintain
the independence of the Older Adult. The Case Managers with the
client and/or family will determine the needs of the individual
through a comprehensive assessment and then develop a service
plan. The service plan will identify programs and services that
will assist the Older Adult. The Case Manager will then communicate
with a variety of community agencies and organizations to arrange
for those services. The Case Managers will assist the clients
and family to assure client's health and safety, the quality
of care and satisfaction with services.
The Case Managers
will advocate for the Older Adult and their family members on
a variety issues. On a daily basis they deal with such issues
as Medicare, Medicaid, supplemental insurance and nursing home
placement.
The Case Management
Program is provided to the Older Adult on a donation basis. If
the client is unable to donate, services will not be denied.
The program is based on need, not on the client's ability to
donate.

The
Homemaking Program is the provision of light household tasks
to maintain an adequate living environment for Older Adults with
functional limitations. Homemaking tasks include: laundry, ironing,
and light housekeeping.
The Homemaking
Program is provided to the Older Adult on a donation basis. The
requested donation is based on a sliding scale according the
clients level of income. If the client is unable to donate, services
will not be denied. The program is based on need, not on the
client's ability to donate.

The Personal
Care Program provides a Certified Nurse Aide to assist Older
Adults with activities of daily living.
These activities include assistance with bathing, dressing, grooming,
toileting, transferring, eating and ambulation.
The Personal

The Respite
Care Program utilizes a Certified Nurse Aide for the provision
of companionship, supervision and/or assistance with activities
of daily living for Older Adults with mental or physical disabilities
in the absence of the primary caregiver(s). This
program
allows "time away" for the caregiver and special companionship
for the frail elder.
The Respite

The Caregiver
Training Program is a service that is designed to provide caregivers
support with the care of their disabled family members. This
training and educational program covers caregivers under the
following categories:
1. Caregivers
60 of older who care for a family member 60 or older
2. Caregivers 60 or older who care for a family member under
60
3. Caregiver
under 60 who care for family member 60 or older.
The training
and educational sessions will take place in the caregivers home
and could include topics such as medication management, how to
care for someone who is one bed rest, fall prevention, locating
resources, managing stressful situations, or differences in dementia,
depression and delirium. The caregivers will be able to select
from these and other topics that may be of interest to them.
Professional from the community will then visit the caregiver
within the home setting to provide information on the topic of
choice.
The Caregiver

The Isabella
County Commission on Aging offers Older Adults medical assistance
with several different programs. Below you will find brief descriptions
of those programs and an idea on the eligibility requirements
needed to qualify.
Medicare Medicaid
Assistance Program (MMAP) -
The MMAP Program is funded by the state of Michigan. This program
provides assistance with questions about Medicare or other insurances,
helps Older Adults find supplemental insurance to their Medicare,
answers questions about insurance through an individual's retirement
program and will help determine if a person is eligible for Medicaid
(medical assistance program through the State of Michigan). There
are no income or asset guidelines for the MMAP Program.
Unmet Needs - This program is totally funded by the
Saginaw / Chippewa Tribal Unit. With it the Commission on Aging
is able to assist Older Adults with expenses such as: prescription
medications, utility bills, minor home repairs or medical expenses.
The Unmet Needs Program is limited to individuals whose income
falls below 150% of the Federal poverty guidelines. Consideration
is also given to the individuals assets and other living expenses.
Mt. Pleasant Area
Community Foundation
- This
fund is only used to provide assistance with prescription medication
costs for Older Adults, age 60 and over living in Isabella County.
The individuals income level must be at or below 150% of the
Federal poverty level. Consideration is also given to the individuals
assets and other living expenses.
Pharmaceutical Assistance
Programs
- Many
of the pharmaceutical companies have patient assistance programs
that help individuals with the cost of prescription medications.
Each company has a different eligibility requirement and application
process. The Commission on Aging Case Managers are familiar with
these companies and will assist in the application process through
this program.
