To obtain a cremation permit through Sparrow Medical Examiner’s office please follow the steps below.
Click on the link below, PAY THE PERMIT FEE, fill in the name of the deceased along with the date of death on the payment notes area. Print the receipt out and fax it with the cremation certificate to the Sparrow Medical Examiner’s Office.
